Clean Claim Rate Calculator
A clean claim is accepted and processed without anyone having to fix, correct or resubmit it. The clean claim rate measures how much of your billing is right the first time, and its inverse is your rework load: every.
What this calculator shows
A clean claim is accepted and processed without anyone having to fix, correct or resubmit it. The clean claim rate measures how much of your billing is right the first time, and its inverse is your rework load: every claim somebody has to touch twice costs staff time and payment delay.
Use your clearinghouse acceptance figures for one period. A claim that was rejected, corrected or denied and resubmitted does not count as clean even if it eventually paid.

Clean Claim Rate Calculator
How many claims go through right the first time, with no rework?
How the calculation works
Clean claim rate = clean claims / total claims submitted x 100. Claims needing rework = total submitted minus clean claims.
How to read your result
The rework count is the operational number: multiply it by the minutes a correction takes and you have the hidden staffing cost of a low clean claim rate. Raising it is mostly front-end work: eligibility and authorization checks, coding review and claim scrubbing, the habits covered in our medical billing KPIs guide and the claims management service.

Before you rely on the number
Results are estimates for educational and operational planning, built from the figures you enter. They are not guarantees of reimbursement, savings or recovery, and they do not replace a review of your own claims, contracts and remittances. Do not enter patient-identifiable or protected health information: this calculator runs in your browser and nothing you type is stored or sent anywhere.
FAQ
Calculator questions
Still have a question? Talk to our team about your billing workflow.
Is clean claim rate the same as first-pass rate?
They are close cousins. First-pass measures claims paid on first submission; clean claim measures claims accepted without correction. A claim can be clean and still be denied later for medical necessity, so track both.
What counts as rework?
Any claim touched again after first submission: rejected and resent, corrected claims, rebills and duplicates created by mistake.
Why does this rate matter if claims eventually pay?
Because rework is paid for in staff hours and slower cash. Two practices with the same collections can have very different billing costs purely from rework volume.
