Psychologist billing services for therapy and testing practices
Fastora provides psychologist billing services for licensed psychologists, testing practices and group practices. Psychology medical billing spans three families of services: diagnostic evaluations and psychotherapy, psychological and neuropsychological testing billed in time units, and health behavior services for patients whose main problem is a physical condition.
Testing is where most of the revenue and most of the risk sit. A single evaluation can span several days of work, involve a technician, use several codes and depend on an authorization that approved a specific number of hours.
Our psychology billing services cover the whole cycle: benefits and authorization checks before testing, coding by certified medical billing and coding professionals, claim submission, payment posting and follow-up on every open claim. Psychology billing for groups adds rendering-provider setup for each psychologist and any technicians who administer tests under supervision.
Therapy and testing codes: psychological testing billing
Testing is split into evaluation services, which cover the psychologist's interpretation and reporting, and test administration and scoring, which is coded by who gave the tests.
| Code | Descriptor (summary) | Unit |
|---|---|---|
| 96130 | Psychological testing evaluation services by a physician or other qualified health care professional | First hour |
| 96131 | Psychological testing evaluation services | Each additional hour |
| 96136 | Test administration and scoring by a physician or other qualified health care professional, two or more tests, any method | First 30 minutes |
| 96137 | Test administration and scoring by the professional | Each additional 30 minutes |
| 96138 | Test administration and scoring by a technician, two or more tests, any method | First 30 minutes |
| 96139 | Test administration and scoring by a technician | Each additional 30 minutes |
| 96146 | Test administration with a single automated, standardized instrument via electronic platform, with automated result only | Per test |
Evaluation services include integrating patient data, interpreting test results and clinical data, clinical decision making, treatment planning, the report and interactive feedback when performed. Time on those activities goes to 96130 and 96131; time spent giving and scoring tests goes to the administration codes.
Under CPT time conventions a unit is reported once its midpoint is passed, so the first hour of 96130 needs at least 31 minutes and the first unit of 96136 needs at least 16 minutes. The clinical interview before testing is usually billed with 90791. Our article on psychological testing billing walks through 96130-96139 with worked examples.
Neuropsychological testing billing: 96132, 96133, 96116 and 96121
Neuropsychological evaluation has its own evaluation codes. 96132 covers the first hour of neuropsychological testing evaluation services by a physician or other qualified health care professional, and 96133 each additional hour. Test administration and scoring still use 96136-96139, coded by whether the professional or a technician gave the tests.
A neurobehavioral status exam is a separate service: a clinical assessment of thinking, reasoning and judgment, such as attention, memory, language and executive function. It is reported with 96116 for the first hour and 96121 for each additional hour, and is often used before deciding whether full testing is needed.
| Code | What it reports | Unit |
|---|---|---|
| 96132 | Neuropsychological testing evaluation services | First hour |
| 96133 | Neuropsychological testing evaluation services | Each additional hour |
| 96116 | Neurobehavioral status exam by a physician or other qualified health care professional | First hour |
| 96121 | Neurobehavioral status exam | Each additional hour |
Payers commonly limit how these codes combine on one evaluation and how many hours they will approve, so we check each claim against the authorization and the payer's billing article before it goes out.
Authorizations for testing
Many commercial plans, Medicaid managed care plans and carved-out behavioral health administrators require approval before testing. Requests typically state the referral question, prior evaluations, the tests planned and the hours requested by code.
Approvals often come back with fewer hours or a different split between evaluation and administration than requested. We compare the approval with the testing plan, flag gaps before testing starts and keep billed units within approved units. See how our prior authorization team handles these requests.
Psychotherapy billing for psychologists
Psychologists bill the same time-based psychotherapy codes as other therapists: 90832, 90834 and 90837 by session length, 90846 and 90847 for family sessions, 90853 for group, and 90785 when interactive complexity applies. Because psychologists do not provide medical E/M services, they use 90791 for diagnostic evaluations rather than 90792, and do not bill the E/M psychotherapy add-on codes.
Our behavioral and mental health billing hub lists the full psychotherapy code set with time ranges, and our medical coding team reviews session codes against documented minutes.
Health behavior assessment and intervention
Health behavior services address psychological and behavioral factors that affect a physical health problem, such as adherence to a diabetes plan, adjustment to chronic pain or preparation for surgery. They are used when the focus is a physical condition; when a mental health disorder is the focus, psychotherapy codes apply instead.
| Code | Descriptor | Unit |
|---|---|---|
| 96156 | Health behavior assessment, or re-assessment | Per service |
| 96158 | Health behavior intervention, individual, face-to-face | Initial 30 minutes |
| 96159 | Health behavior intervention, individual, face-to-face | Each additional 15 minutes |
Group and family versions of the intervention codes also exist. Payers differ on which license types may bill these services, so we confirm coverage for the rendering clinician before sessions are scheduled.
Documenting time for testing and therapy
Nearly every code on a psychology claim depends on time, which makes time documentation the main audit exposure. Notes should separate minutes by activity:
- Test administration minutes by date, and whether the psychologist or a technician gave the tests
- Evaluation, report-writing and feedback time
- Total face-to-face psychotherapy minutes, with start and stop times where payers expect them
- Tests given and the scoring method, including any automated instruments
Testing often spans several dates. Payers differ on whether each service is billed on the date performed or the evaluation codes on the date the report is finished, and we follow each payer's convention.
Medicare and commercial considerations for psychology billing
Medicare enrolls clinical psychologists as their own provider type and covers psychological testing when it is medically necessary and documented. Medicare Administrative Contractors publish local billing and coding articles listing covered diagnoses and documentation expectations, and technician administration is billed under the supervising psychologist.
Commercial plans may exclude testing done mainly for educational or vocational purposes, cap hours per evaluation or require a medical referral question. Some limit testing by age group or diagnosis. We check these limits during coverage verification, before the first testing appointment.
Credentialing for psychologists
A psychologist must be enrolled with each plan, and often with a carved-out behavioral health administrator, before testing or therapy claims pay. Technicians who administer tests are generally not enrolled separately; their work is billed under the supervising psychologist, so the psychologist's enrollment has to be complete first.
Fastora follows a tracked nine-stage process: consultation, document collection, state licensing, NPI and CAQH setup, payer applications, panel submissions, follow-ups, final approval and ready to bill. See our credentialing and payer enrollment service for details.
Psychology medical billing: how Fastora supports your practice
Certified coders check testing claims against documented minutes and approved hours through Advanced Pre-Submission Claim Review. Our team posts payments, works denials and follows up on unpaid testing claims, which often carry larger balances than therapy sessions.
Because testing claims often combine four or more codes, one unit error can hold up payment on the whole evaluation. We compare billed units with the approval and the note on every testing claim, and send corrected claims quickly when a payer adjusts units.
Practices get a dedicated account contact, transparent dashboards and a monthly review call, with custom reports on request, such as testing hours billed compared with hours authorized. Fastora quotes after a free billing assessment, with no hidden setup fees; our pricing page explains the models we offer.
Insurance billing for psychologists
Insurance billing for psychologists is session-based: timed psychotherapy codes, diagnostic evaluations, testing when billed, and visit-limit tracking against carve-out authorizations. A psychology billing company should check benefits against the behavioral administrator — not just the medical plan — track authorizations by visit count, and keep each note tied to the treatment plan. Telehealth sessions add their own rules: place of service, modifiers, and state licensing for the patient's location. See our therapist billing page for the private-practice workflow.
Psychologist credentialing
Psychologist credentialing means CAQH profile setup and attestation, NPI registration, and applications to each commercial panel plus Medicare and Medicaid where applicable — with extra attention to whether a behavioral health carve-out runs its own panel. Every application is tracked separately, and revalidation dates go on a calendar instead of in someone's memory, because a lapsed enrollment stops claims from paying. Our mental health credentialing guide walks through the timeline and read insurance paneling for therapists.
