What this calculator shows
The in-house side of the outsourcing question is usually understated, because the cost is spread across salaries, software invoices and other people's time. This calculator asks you to name each line once, for a year, then compares the total with an outsourced fee calculated from your own collections, either as a percentage or a flat monthly fee.
Enter either the percentage or the flat fee, not both. If you enter both, the flat fee is used. This is an estimated cost comparison: it does not claim outsourcing is always cheaper, and it does not model changes in collections, which are covered by the ROI calculator.
Revenue & ROI calculator
Medical Billing Outsourcing Cost Calculator
Add up your real in-house billing cost, line by line, against an outsourced fee.
How the calculation works
In-house cost is the sum of your annual lines. Outsourced cost is monthly collections x fee percentage x 12, or flat fee x 12. Cost to collect divides each annual cost by annual collections (monthly collections x 12).
How to read your result
Cost is only one side of the decision. The other side is what each arrangement collects: denial follow-up, underpayment checks and A/R work differ more between teams than fees do. Read this result together with the medical billing ROI calculator and our plain-language comparison of in-house vs outsourced medical billing.
If you want the comparison run on your real figures, a free medical billing audit starts from your claims and A/R rather than estimates, and Fastora's pricing page explains how a written quote is built.

Before you rely on the number
Results are estimates for educational and operational planning, built from the figures you enter. They are not guarantees of reimbursement, savings or recovery, and they do not replace a review of your own claims, contracts and remittances. Do not enter patient-identifiable or protected health information: this calculator runs in your browser and nothing you type is stored or sent anywhere.
FAQ
Calculator questions
Still have a question? Talk to our team about your billing workflow.
Does outsourcing always cost less?
No. A small practice with a stable, experienced biller can be cheaper in-house. This calculator exists so you can see your own numbers instead of relying on either side's sales claims.
What do practices forget to count in-house?
Management time, covering for vacations and turnover, re-training after payer rule changes, and the cost of denials that age while a position is empty. The lines above prompt for most of them.
Should I enter a percentage or a flat fee?
Whichever a written quote gives you. Percentage fees scale with collections; flat fees are predictable. Enter only one so the comparison stays clean.
Want this run on your real claims?
A free billing assessment reviews your actual denials, A/R and contracts, then gives you these numbers from your own data, in writing, before you decide anything.
