Medical Billing Services in Denver, Colorado

Denver anchors Colorado’s practice market: group practices spread across Denver, Aurora, and the southern suburbs, therapy and orthopedic clinics serving an active Front Range population, and behavioral health practices in every part of the metro. What ties their billing together — and trips it up — is a Medicaid structure that sends neighboring claims to two different destinations.

Fastora Medical Billing Solutions handles that work for Denver practices: claims, denials, eligibility, credentialing, and the follow-up that keeps collections steady. This page is about the Denver metro specifically. For the national picture, see our page on how Fastora bills for practices in all 50 states.

One Medicaid program, two claim destinations

Colorado’s Medicaid program is Health First Colorado, administered by the Department of Health Care Policy and Financing (HCPF). Care is organized through the Accountable Care Collaborative (ACC), and since July 1, 2025 — the start of ACC Phase III — the state has been divided into four regions, each run by a Regional Accountable Entity, or RAE. Every member is assigned to a RAE based on where their primary care provider is located, or on their home address if they have none.

The Denver metro’s RAE is Colorado Access, covering Denver, Adams, Arapahoe, and Douglas counties. The RAEs administer the capitated behavioral health benefit, which produces the split every Denver billing office has to live with: behavioral health claims go to the member’s RAE — Colorado Access, for this metro — while physical health Medicaid claims are submitted to the state through the Provider Web Portal. The same practice, on the same day, can owe claims to both.

There is a second wrinkle inside the metro. Some Denver-area members are enrolled in Elevate (Denver Health) Medicaid Choice, a managed care plan, and Colorado Access manages behavioral health for those members too. HCPF’s guidance to behavioral health providers is direct: where a service might otherwise be billed to the state fee-for-service, a managed-care member’s claim goes to the managed care organization instead. Eligibility checks that stop at “Medicaid is active” miss exactly the detail that decides where the claim belongs.

What the split does to a Denver billing office

The July 2025 transition is still working its way through billing offices. When Phase III launched, members were reassigned to the new RAEs under new boundaries and the RAEs rebuilt their provider networks. Practices still find members whose RAE assignment changed under them, and claims that went to a destination that was right last year and is wrong now.

Two requirements sit underneath the routing. First, enrollment: providers enroll with Health First Colorado through the Provider Web Portal — the state interChange system, processed by HCPF’s fiscal agent, Gainwell Technologies — and register their assigned provider ID before billing. A new provider, or a new location in Aurora or Centennial, that is not correctly enrolled cannot bill anyone. Second, contracting: state enrollment alone does not let a behavioral health practice serve RAE members. A behavioral health provider who is not contracted with the member’s RAE cannot bill for those members without a contract or a single case agreement — for Denver metro members, that means a Colorado Access contract has to be in place.

And the clock is unforgiving in a specific way. Health First Colorado claims must be filed within 365 days of the date of service. Claims that need to stay alive past that point have to be resubmitted every 60 days, with the previous claim’s Internal Control Number referenced — a discipline that only works if someone tracks it claim by claim.

What Fastora handles for Denver practices

Fastora can run the whole billing cycle for a Denver practice, or only the pieces your team cannot get to:

  • Claims submission and follow-up. Claims are prepared, checked, and sent to the right destination — payer, the state portal, or the member’s RAE — then tracked until each one is resolved.
  • Denial management and appeals. Denials are worked by reason — corrected, resubmitted, or appealed where the documentation supports it — instead of aging in a queue.
  • Eligibility verification. Coverage is confirmed before the visit, including the member’s RAE assignment and any managed care enrollment, such as Elevate Medicaid Choice.
  • Prior authorization. Approvals are requested and tracked so recurring care does not run past the authorization on file.
  • Credentialing and enrollment. Providers and locations are set up in state enrollment through the Provider Web Portal and in payer and RAE records, so growth does not stall at the billing office.
  • Accounts receivable recovery. Aged claims are reviewed and worked in order of recoverability — including the resubmission discipline that keeps older Health First Colorado claims in timely filing — and the causes behind them are fixed so the backlog does not rebuild.
  • Payment posting and reconciliation. Payments and remittances are posted and reconciled, which is also how underpayments surface when a payer pays below the contracted rate.

Fastora has worked in medical billing for 5+ years, serves 50+ active clients, and has supported 100+ satisfied MDs. The work is priced simply: a percentage of collections or a flat monthly fee, and during the initial audit we will compare both against your current billing costs.

Specialty billing in the Denver market

Orthopedics and sports medicine. Denver’s orthopedic groups bill across the full range — fracture care, joint work, sports injuries from a population that stays active year-round — with procedure coding, modifiers, and authorizations that have to line up before claims go out. See how we handle orthopedic billing services for practices where surgical and clinic volume run side by side.

Physical therapy and rehabilitation. Therapy billing turns on timed units, visit limits tracked against plans of care, and authorizations kept current across recurring treatment — many small claims where consistency protects the whole month. Our physical therapy billing services page covers how we manage therapy practices’ billing.

Behavioral health and psychiatry. This is where the Denver Medicaid structure bites hardest. Session coding has to match the documentation, authorizations have to stay current across recurring care, and the claim’s destination depends on the member’s RAE assignment or managed care enrollment — with a Colorado Access contract needed before any of it pays. It is the specialty where eligibility and enrollment work matters most, and we set those workflows up with you at the start.

Pain management. Pain practices combine procedure billing with recurring visit care, where authorization and documentation gaps repeat across every cycle.

Frequently asked questions

Where do Medicaid claims go for a Denver practice?

It depends on the service. Physical health claims for Health First Colorado members are submitted to the state through the Health First Colorado Provider Web Portal. Behavioral health claims go to the member’s Regional Accountable Entity — in the Denver metro (Denver, Adams, Arapahoe, and Douglas counties) that is Colorado Access. Sending a claim to the wrong one of the two is a routing denial — one of the most common Medicaid errors in this market.

What changed with ACC Phase III in July 2025?

Colorado consolidated its Medicaid regions from seven to four, every member was reassigned to a Regional Accountable Entity under the new boundaries, and the RAEs rebuilt their provider networks under new contracts. In practice, a member’s RAE may be different from the one a practice billed before, and behavioral health contracts may need to sit with a different entity. Assignment follows the location of the member’s primary care provider.

How long do we have to file Health First Colorado claims?

365 days from the date of service. If a claim needs to remain open beyond that, it has to be resubmitted every 60 days with the previous claim’s Internal Control Number referenced, or it falls out of timely filing. Commercial payers and Medicare Advantage plans set their own deadlines in their contracts, so the Medicaid window is not the only clock a practice runs on.

Does our behavioral health practice need a contract with Colorado Access?

Yes, if you serve Health First Colorado members in the Denver metro. State enrollment makes you a Health First Colorado provider, but a behavioral health provider who is not contracted with the member’s RAE cannot bill for that member’s care without a contract or a single case agreement. Colorado Access is the RAE for Denver, Adams, Arapahoe, and Douglas counties.

We are adding a provider or a new location. What does billing need?

The new provider or location has to be enrolled with Health First Colorado through the Provider Web Portal — enrollment is processed by the state’s fiscal agent, Gainwell Technologies — and added to your commercial payer and RAE setups before claims connected to it go out. Claims sent before that groundwork is in place deny like any other error, so enrollment belongs on the opening checklist, not after it.

What does Fastora charge?

You choose between a percentage of collections and a flat monthly fee. Which fits better depends on your practice’s size and how much of the cycle you hand over; in the initial audit we price both against your current costs so you can compare real numbers.
