Speech Therapy CPT Codes Guide

Speech-language pathology evaluation codes

Evaluations of speech fluency, speech sound production, voice and resonance, and language comprehension are billed with 92521 through 92524. Like other therapy evaluations they are untimed and reported once per evaluation, with the report carrying the clinical findings.

Most SLP treatment is billed with 92507 (individual treatment of speech, language, voice or auditory processing). Because 92507 is untimed under Medicare, minutes do not create extra units the way timed PT and OT codes do; the 8-minute rule explains where timed counting applies and where it does not.

The GN modifier and plan rules

Medicare claims for speech-language pathology services carry the GN modifier. Plans of care, recertification and visit limits are tracked the same way as other outpatient therapy, and the threshold rules in our KX modifier guide apply to SLP spending as well.

Fastora's speech therapy billing services page describes how authorizations, school-based referrals and medical claims are coordinated for SLP practices.

Documentation that supports SLP claims

Notes should show the deficit treated, the skilled intervention, the client's response and progress toward measurable goals. Vague session notes are a common reason SLP claims are questioned on review.

Where documentation or coding is the problem, medical coding review and the denial patterns in how to reduce claim denials are the fastest places to start. Group practices can also compare rehabilitation billing for multi-discipline rules.

Frequently asked questions

Is CPT 92507 timed?

Under Medicare, 92507 is an untimed, service-based code billed once per session regardless of length. Some commercial plans set their own session rules, so check the contract.

What does the GN modifier mean?

GN identifies services delivered under an outpatient speech-language pathology plan of care on Medicare claims.
